Athlete Registration

 

The South Side Athletic Department is excited to announce that we are now offering the convenience of online athletic registration through Final Forms.

 

Final Forms is a secure registration platform that provides you with an easy, user-friendly way to register for athletics, and helps us to be more administrativelyefficient and environmentally responsible. When you register through Final Forms, the system keeps track of your information in your FamilyID profile. You enter your information only once for each family member for multiple uses and multiple programs.

 

BEFORE YOU REGISTER:

Prior to registration, the form should be uploaded to your Final Forms account. A health exam is valid for a period of 12 months - through the last day of the month in which the health exam was conducted.

 

REGISTRATION PROCESS:

A parent/guardian should register by clicking here.

The registration portal will open 30 days prior to the first day of each season's tryout.

 

Fall Season 

  • Registration opens for High School Football & Cheerleading: July 17th 2026
  • Registration opens for High School Fall Sports: July 24th 2026
  • Registration opens for Middle School Sports: August 3rd 2026


Winter Season 

  • Registration opens for High School Winter Sports: October 16th 2026
  • Registration opens for Middle School Winter 1 Sports: October 2nd 2026
  • Registration opens for Middle School Winter 2 Sports: December 21st 2026


Spring Season 

  • Registration opens for High School Spring Sports (Except Girls Golf and Boys Tennis): February 8th 2027
  • Registration opens for High School Girls Golf and Boys Tennis: February 15th 2027
  • Registration opens for Middle School Spring Sports: March 1st 2027

 

Please note the following steps must be completed to ensure the nurse will be able to review and clear the student for his/her chosen sport:

  1. Make sure to add sport
  2. Complete all forms

All parents/guardians and athletes must electronically sign the Concussion Acknowledgement and Student-Athlete Contract in Final Forms. This signature confirms their understanding of concussion signs and symptoms, as well as the expectations for a South Side student-athlete.

Once a parent/guardian signs these documents, the athlete will receive an email with instructions on how to provide their electronic signature. Athlete clearance is contingent upon their electronic signature.

Once everything is completed and signed, the nurse will be able to review/clear your child for sports.
